Summary of Essential Functions for the Position: A Home Health Aide (HHA) is specifically trained, competent and performs the provisions of hands-on personal care, the performance of simple procedures as an extension of therapy or nursing services, assistance in ambulation exercises, assistance in administering medications ordinarily self-administered (assistance is limited to getting water or fluids) to the patient in their residence under the direction, instruction and supervision of the registered nurse (RN). The Skilled professional staff whether direct employee of the agency or under arrangement are expected to contribute to all phases of the QAPI program and participates in with ongoing interdisciplinary assessments of patients. The HHA will participate in the development and revision of the plan of care in partnership with the patient, patient representative (if any) and caregiver(s).
